Preventive services must be covered up front with no cost-sharing and no annual limits.
Overview
Section 2713 of the Public Health Service Act, as added by the Patient Protection and Affordable Care Act, requires coverage without cost sharing of certain preventive health services, including certain contraceptive services, in non-exempt, nongrandfathered group health plans and health insurance coverage.
This requirement was effective for non-grandfathered plans with the first renewal on or after September 23, 2010. In total, there are 15 covered preventive services for adults, 22 covered preventive services for women, including pregnant women, and 26 covered preventive services for children. More info here.
One of the more controversial requirements is that plans in the Health Insurance Marketplace cover contraceptive methods and counseling for all women, as prescribed by a health care provider. These plans must cover the services without charging a copayment, coinsurance, or deductible when they are provided by an in-network provider. There is no requirement to cover rugs to induce abortions or to cover services related to a man’s reproductive capacity, like vasectomies. More info here.
